DISTINGUISHED SERVICE AWARD
Nominations for the Northwest Planned Giving Roundtable Distinguished Service Award are accepted through May 31st.
The Northwest Planned Giving Roundtable Distinguished Service Award was established in 2008 to recognize individuals who have exemplified exceptional commitment and dedication to the planned giving effort and community.

Please submit a letter of recommendation for the individual you would like to nominate for this award along with this form. Only one nomination may be made in one year by any individual.
Applications are available as of April 1st each year and must be submitted (postmarked by U.S. mail or via email) by May 31st to be considered. Please remit the application and cover letter via U.S. mail to:
